It will ask you to upload 3 images of yourself in a natural position focusing on the area of interest. This form is HIPAA secure and images will only be seen by our staff. In most cases, it is best to have someone take these photos for you. After reviewing your form, our office staff will reach out to you to schedule an online consultation. You will be emailed a secure link to connect with our team via a HIPAA-compliant video conferencing tool at a scheduled appointment time.
